Description of module content:

Adult Shortened Programme for Graduates (SPG) and Conversion Course (CC) students will gain community experience, reflection on and in practice, exploring the role of the primary health care team in supporting patients, families and the community. The organisation of care within a community setting will be explored including e-health and public health. Preparation for Practice (PfP) will include lectures and online materials on wound management, dermatology, nutrition for patients with complex needs, palliative care, genetics, assessment and continence promotion and Cleanliness Champions Package (CCP) learning units.

Learning Outcomes for module:

Upon completion of this module you will be able to
LO1: Deliver, under direct / indirect supervision compassionate, person/ family centred care which embeds the principles of holistic practice, respecting the diversity of client groups (NMC, 2008).
LO2. Demonstrate an evidence based approach to practice in a variety of heath care settings within the community, utilising local, national and international guidelines and journals
LO3. Contribute in a range of nursing skills to meet the holistic needs of patients, ensuring patient/ client safety and well being at all times.
LO4: Discuss the partnerships and collaboration of the primary health care team in caring for clients with varied needs, and the organisation of care in the community
LO5: Apply knowledge of infection control interventions to clinical practice
LO6. Critically reflect on the health assessment process and the needs of the local population
