Students will select a specific long term condition to critically examine the evidence base underpinning current practice in managing psychosocial issues and promoting self-management (LOs1, 2, 3). A person centred evidence-based approach will underpin the teaching and learning activities to promote best practice, to meet the needs of the people living with a long term condition (LO 4). The module will develop the students’ knowledge and skills to adopt an enabling role and work in partnership with client/carers and interprofessional working, within health sectors (LOs1 to 4).The module will be delivered fully online via Moodle (LOs 1 to 4). A variety of teaching strategies will be employed, designed to enhance the learning experience by encouraging the student to engage with the material in a meaningful way. The module content will be delivered in units to guide students through the module in a structured manner (LOs 1 to 4). Collaborative learning will be promoted through the use of synchronous and asynchronous online discussion groups on Moodle facilitated by the module team (LOs 1 to 4). The use of Moodle in this module gives greater flexibility so students can choose to learn at a time which suits them. Online tutorials, lectures and the use of WebEx will offer the opportunity for students to get together with fellow students in small groups to learn through interactive teaching sessions which aim to enhance and extend students learning (LOs 1 to 4). The activities will encourage students to engage with the literature and critically reflect on practice (LOs1 to 4).

Students will submit an essay patch of 750 words outlining their proposed summative assessment in week 7. Individual feedback will be given.

Week 14 - The summative (marked) assessment will be in the form of a portfolio of elements – a written report.A 3500 word portfolio on their chosen long term condition. Students will compare their current practice, in supporting a person to manage the selected psychosocial issue, to evidence for best practice. Students will make recommendations for future practice.
