This module will give you the opportunity to develop skills to comprehensively assess patients’ health and well-being and use this to formulate an evidence-based plan of care. To enhance your knowledge of patient assessment, you will be asked to consider theory related to patient assessment and the evidence-base that aligns with this. This will support you to systematically and comprehensively assess patients’ physical health and well-being. You will also enhance your knowledge of how to take a comprehensive full clinical history of patients’ health. You will learn how to interpret data gained from patients’ history and health assessments to formulate an evidence-based plan of care and enhance clinical decision making. Patient scenarios will be used to help adopt a holistic approach to gathering a full clinical history, a systematic assessment, and interpret the assessment findings to make an evidence-based plan of care. The patient scenarios used in the module will relate to variety of healthcare environments (e.g. secondary care/primary care / long-term care/ rehabilitative care and community-based settings) to reflect the diverse range of environments where patients have their physical and general health assessed. The patient scenarios will be based on common conditions that reflect the global health challenges.
