Description of module content:

In this module students will learn about the consequences that living with a chronic disease (or diseases) has on the individual, as well as the impact on family, friends, and carers. Students will focus their learning on a particular chronic disease pertinent to their professional practice that is recognized globally, nationally or locally as a health priority. They will then use an evidence-informed approach to explore the role of the nurse in the enhancement of patient outcomes and the development of effective self-management strategies.

Learning Outcomes for module:

Upon completion of this module you will be able to

LO1: Critically evaluate the evidence-base underpinning current practice in the management of people affected by chronic diseases.

LO2: Evaluate the social and psychological impact of conclusive diagnosis of a chronic disease on people and their family/ carers.

LO3: Critically appraise the role of the multidisciplinary team in promoting self-management to enable people to live positively with a chronic disease.

LO4: Evaluate current local practice and possible changes to improve psychological symptoms and health, and well-being for people living with one or more chronic diseases.
