Description of module content:

In this module you will learn about how living with a chronic disease (or diseases) may impact the individual, their family, friends, and carers. You will be invited to focus on one chronic condition pertinent to your clinical practice that is recognized globally, nationally or locally as a health priority. You will then use an evidence-informed approach to explore the role of the nurse in the enhancement of patient outcomes and the development of effective self-management strategies.

Learning Outcomes for module:

Upon completion of this module you will be able to

LO1: Critically evaluate the evidence-base underpinning current practice in the management of people affected by chronic diseases.

LO2: Evaluate the social and psychological impact of conclusive diagnosis of a chronic disease on people and their family/ carers.

LO3: Critically appraise the role of the multidisciplinary team in promoting self-management to enable people to live positively with a chronic disease.

LO4: Evaluate current local practice and possible changes to improve psychological symptoms and health, and well-being for people living with one or more chronic diseases.

Face To Face Lecture 15 The teaching approach for this module is blended with face-to-face in person classes, independent groupwork and guided independent study. The teaching activities will include scheduled lectures and tutorials in which the focus will be on the ways in which nurses can support individuals and their families to live well with a chronic disease (LO 1, 2, 3 & 4).
Face To Face Tutorial 20 To support your learning journey, the teaching schedule will include (2 hour) interactive tutorials which will occur weekly immediately following each lecture in which you can further explore key concepts (LO 1, 2, 3 & 4). In addition, to tutorial sessions, there will also be student and academic forums online to provide you with additional opportunities to discuss key concepts and seek individualised support.
Online Guided independent study 145 The learning platform will be used to support your guided independent study (weekly schedule) and provide activities that will guide you through the module in a structured manner. These will include online learning materials, podcasts, quizzes, scenario work and multi-media resources that encourage your engagement through a discursive and interactive approach to learning. In addition, group forums will help consolidate your learning and facilitate an opportunity for peer-peer feedback.
Online GROUPIND_STUDY 20 During this module, you will be provided the opportunity to work co-productively with your peers. In small groups you will have an opportunity to meet and discuss the key concepts on the module and explore the role of the nurse in supporting patient's and their families to live well with chronic disease. The opportunity to consider a range of chronic diseases will broaden your understanding of assessment tools, nursing management, health affirming approaches, and other therapies.

Report 100 1~2~3~4 Week 13 , WORDS= 1500 The summative (marked) assessment will be a person centered and evidence-based report. You will select one chronic disease and write a report illustrating and examining issues that may affect a person living with this disease. In your report you will consider how the nurse can support the individual in the development of self-management strategies and enhance their independence and quality of life.
